Discussion was held on the topics of current enrollment, current staffing, and background checks.
The following inspection took place between the hours of 10 am through 3 pm. Today, I met with the Staff of Building Blocks II Day Care located in Falls Church, Virginia. At the start of this unannounced monitoring inspection there were a total of 33 children with 8 staff; good ratios in all 4 rooms. A total of 7 children?s files and 13 staff files were reviewed. The children were engaged in the following: story time, infant sleeping, infant feeding, diapering, bathroom breaks, hand washing, large motor activities outside on center playground, toy play, games, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-160-A Description: Based on review, 1 of 13 staff files reviewed indicated that each staff member did not submit documentation of a negative tuberculosis (TB) screening no later than 21 days after employment. Evidence - 1. On the date of inspection (03/13/2019), TB documentation was not available for review for Staff #8. 2. The start date for Staff #8 was listed as 02/11/2019. Plan of Correction: TB documentation shall be obtained for Staff #8 and placed on file.
Standard #: 22VAC40-185-160-C Description: Based on review, 1 of 13 staff files reviewed indicated that it has been more than two years from the date of the most recent tuberculosis (TB) documentation of file. Evidence - 1. On the date of inspection (03/13/2019), current TB documentation was not available for review for Staff #9. 2. The date of the TB documentation on file for Staff #9 was listed as - 11/12/2015. Plan of Correction: Updated TB documentation shall be obtained for Staff #9, and placed on file.
Standard #: 22VAC40-185-70-A Description: Based on review, 2 of 13 staff files reviewed indicated that not all of the required information was contained within each staff's record. Evidence - 1. On the date of inspection (03/13/2019), 0 of 2 references were on file for: Staff #7, and for Staff #12. 2. The start date for Staff #7 was listed as - 02/25/2019. The start date for Staff #12 was listed as - 07/11/2018. Plan of Correction: All missing information shall be obtained and placed on file.
Standard #: 63.2(17)-1720.1-A Description: Based on review, 1 of 13 staff files reviewed indicated that not all staff obtain the results for a fingerprint background check for the licensed child day center prior to their employment. Evidence - 1. On the date of inspection (03/13/2019), the results of a fingerprint check reviewed for Staff #10 revealed that the "eligbility letter" dated 08/24/2018 was issued for another center. Although this center is a "sister" center to the employer of Staff #10, its sponsorship was under a different LLC. and is not transferable between the two. 2. During the initial inspection (02/28/2018), the center was instructed that separate background checks would be required for each center. 3. The start date for Staff #10 was listed as 09/04/2018. 4. Staff #10 was on site working during the date of inspection. Plan of Correction: Staff #10 has scheduled to have a fingerprint check. Once the results are in the Licensing Inspector shall be contacted for review. In the meantime, Staff #10 will not be permitted to work at this center.
