An unannounced monitoring inspection and facility tour was conducted from 12:00pm-1:00pm. At the time of inspectors' arrival there were 10 infants to two year olds in care with 3 staff members. Children were observed participating in infant activities, interacting with staff, feeding, diapering, transitioning between activities, and nap. A sample of 5 children's records and 5 staff records were reviewed. An injury report was also reviewed. The Staff reported that there are no medications being administered at this time. Areas of noncompliance are detailed in the violation notice and were discussed with the Director at the conclusion of this inspection.

Standard #: 22VAC40-185-60-A
Description: Based on record review it was determined the center did not ensure that each child's record contained all of the required components. Evidence: 1. The record for child #3 did not contain the city, state, and zip code for the two required components and any allergies. 2. The Director confirmed that the information was not present during the inspection.

Plan of Correction: Director will request the information from the parent at pick up.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview it was determined the center did not ensure that each staff record contained all of the required components. Evidence: 1. The record for staff #1(hired 3/13/19) did not contain: -the two required references -documentation of orientation 2. The record for staff #5(hired 12/11/17) did not contain: -emergency contact -the two required references 3. The Director confirmed that this documentation was not present at the inspection.

Plan of Correction: The Director will ensure that each staff record contains all of the required components.

Standard #: 22VAC40-185-260-B
Description: Based on record review and interview it was determined that the center did not receive annual approval from health department. Evidence: 1. The center did not have current approval from the local health department. The documentation available was dated 8/8/17. 2. The Director confirmed there was not current health department approval.

Plan of Correction: The Director will schedule an inspection with the local health department.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center was maintained in a clean, safe, and operable condition. Evidence: 1. The shelf in the two year old room rocked when pulled and therefore needs to be anchored to the wall. 2. The heaters along the walls in the infant room were measured at 107.4 degrees Fahrenheit. 3. The areas noted above were accessible to children in care and presented potential injury hazards.

Plan of Correction: The shelf will be anchored and barriers will be placed in front of heaters to prevent access.
