Standard #: 22VAC40-185-270-A Description: Based on observation, not all of the areas and equipment on the outside of the center were not maintained in a safe and operable condition. Evidence: The key was left inside the electronic panic bar lock on the fence door located on the toddler playground. The electronic panic bar was not functioning properly as it was not able to be locked and the key removed without the alarm sounding. Plan of Correction: There is a slide lock on the fence door, it was locked and secured during the inspection. The maintenance technician was contacted during the inspection regarding the electronic panic bar lock. The center will keep the children off the playground until the lock is fixed.
Standard #: 22VAC40-185-350-A Description: Based on observation and staff interview, the center did not maintain the required staff-to-child ratio when directly supervising children. Evidence: On March 26, 2019, there were eleven three and four year old children in the care of one staff member. There was a two year old child that was unable to go into a classroom due to staff-to-child ratio from 7:15 AM to 7:20 AM. Plan of Correction: The center will make changes to staff schedule to cover when children arrive to the center during morning hours.
