Comments:
An unannounced renewal inspection was conducted on March 26, 2019 from approximately 10:30 AM to 1:30 PM to determine the facility's compliance with licensing standards. The staff to child ratios were observed as follows: Infants 2:8, Toddlers 1:5, 2 year olds 2:12, 3 year olds 2:17, 4 year olds 1:10. Lunch today consisted of a cheeseburgers on buns, potato wedges, diced pineapple and milk. All areas used for child care were inspected. The facility has age appropriate materials and supplies. The following required information was posted: the license, most recent inspection notice, emergency numbers, evacuation map/procedures, daily schedules, and various parent information. The first aid kits and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Five staff records and five children's records were reviewed. There were no medications at the center at the time of the inspection. Staff on site have current First Aid, CPR, and DHO training. Multiple staff have MAT certification. The most recent evacuation drill was documented on 2/14/19. The most recent shelter-in-place drill was documented on 2/27/19. Standard #: 22VAC40-185-130-A
Description: Based on review of five children's records, two children's records did not contain documentation of immunizations in the required timeframe. Evidence: The licensing inspector observed the records for Child #4 (start date 6/12/17) did not contain documentation of immunizations. The record for Child #5 (start date 5/4/18) contained an immunization record dated 9/26/18.Documentation of immunizations are required to be obtained prior to a child attending the center.

Plan of Correction: Administration will speak with parents and ensure records are updated.

Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, three children's records did not contain documentation of a physical examination by or under the direction of a physician within the required timeframe. Evidence: The licensing inspector observed the records for Child #4 (start date 6/12/17) and Child #3 (start date 9/4/18) did not contain documentation of a physical exam. The record for Child #5 (start date 5/4/18) contained documentation of a physical exam dated 9/26/18. Documentation of a physical exam is required to be obtained prior to or within the first 30 days of a child attending the center.

Plan of Correction: Administration will speak with parents and ensure records are updated.

Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records, the licensee did not ensure that one of five staff records reviewed contained documentation of an updated tuberculosis screening which is required at least every two years. Evidence: The record of Staff #2 showed a record of last having a tuberculosis screening on 3/8/2017.

Plan of Correction: Administration will ensure the updated tuberculosis test results are documented in the staff record.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the center did not ensure that two records contained the required information. Evidence: The record for Child #3 (start date 9/4/18) did not contain a second emergency contact or an address for the listed emergency contact. The record for Child #5 (start date 5/4/18) did not contain addresses for the two emergency contacts or proof of ID and age for the child.

Plan of Correction: Administration will speak with parents and ensure the records are updated.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of five staff records, the facility did not obtain results of a national fingerprint-based criminal history record check for four staff members in the required timeframes. Evidence: 1. The record for Staff #1 indicated a hire date of 1/2/07 and results from the national fingerprint-based criminal history record check dated 10/1/18. 2. The record for Staff #2 indicated a hire date of 4/20/94 and results from the national fingerprint-based criminal history record check dated 10/1/18. 3. The record for Staff #3 indicated a hire date of 3/1/16 and results from the national fingerprint-based criminal history record check dated 10/1/18. 4. The record for Staff #5 indicated a hire date of 4/16/98 and results from the national fingerprint-based criminal history record check dated 10/1/18. For staff employed prior to 1/22/2018, a statement of eligibility based on a national fingerprint-based criminal history record check was required to be obtained prior to 9/30/2018.

Plan of Correction: The center will ensure that timeframe requirements for background checks are met in the future.
