63.2 Facilities & Programs.
The licensing inspector conducted an unannounced renewal inspection on Wednesday, March 27, 2019 from approximately 1:00 pm to 5:00 pm to determine the center's compliance with licensing standards. The children in care were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 132 children in care and 21 staff. There were additional staff on site for administrative and support purposes. During the inspection, the licensing inspectors observed the children in care completing a variety of activities to include completing bathroom breaks, napping, participating in circle time and story time, working on an adding pennies activity, and engaged in outdoor play. The center's classrooms, bathrooms, and outdoor playground areas were inspected. During the inspection, the following information was reviewed by the licensing inspector: daily attendance, emergency supplies, injury reports, fire and shelter-in-place drills, liability insurance policy. There were six children?s records and five staff's record reviewed during this inspection. The required posting were inspected. Medication is being administered, medications, and written authorizations were reviewed for the center's compliance. The Department has received a complete renewal application and annual fee from the center. Standard #: 22VAC40-185-160-A Description: Based on review of staff record and interview with staff, the center did not ensure all staff submit documentation of a required tuberculosis screening. Evidence: During the inspection on March 27, 2019, in Staff#1's record (date of hire: 07/05/18) documentation of a tuberculosis screening was not found. In Staff#5's record (date of hire: 10/18/18) documentation of a tuberculosis screening was not found. Interview with staff confirmed documentation for each staff was not available during the inspection. Plan of Correction: Staff#1 will obtain TB screening immediately. Staff#5 will obtain TB screening before returning to work. (04/06/19)
Standard #: 22VAC40-185-70-A Description: Based on review of staff records and interview with staff, the center did not ensure to obtain all required information. Evidence: During the inspection on March 27, 2019, the following information was missing from staff records: 1. In Staff#1's record (date of hire 07/05/18), there was was one reference documentation found checked before employment or volunteering. 2. In Staff#4's record (date of hire 3/16/18),written information to demonstrate orientation training was not found. 3. In Staff#5's record (date of hire 10/18/18) there was was one reference documentation found checked before employment or volunteering. Plan of Correction: References for Staff#1 & 5 will be obtained immediately. Orientation information for staff #4 will be obtained immediately.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of staff records and interview with staff, the center did not ensure to submit Office of Background (OBI) fingerprinting for all staff by required deadline. Evidence: During the inspection on March 27, 2019, in Staff#5's record (date of hire 10/18/18) there was not a determination of results of an OBI fingerprinting found. Interview with staff confirmed that OBI fingerprinting results was not available during this inspection. Plan of Correction: Staff#5 will obtain OBI fingerprinting before returning to work.
