22VAC 40-185-210 .A.4 standard was reviewed.
An unannounced Monitoring Inspection was conducted on March 27, 2019 from approximately 7:45 am until 12:45 pm. Upon entrance there were 43 children and 10 staff present with children. Additional staff were on site for administrative purposes. The record review sample consisted of 6 children's records and 5 staff records. Children were observed preparing for and eating breakfast, reading books and playing with table top toys. Children were also observed playing outside. The infants were observed eating and freely playing. One medication was on site and reviewed. Areas of noncompliance are noted on the violation notice. Standard #: 22VAC40-185-270-A Description: Based on observation and measurements , the licensee did not ensure the areas of equipment of the center inside, was maintained in a safe and operable condition. Evidence: On March 27, 2019, on the 3- 5 year old playground there is a multicolored plastic caterpillar with broken area and missing the plastic eye and this area has sharp edges and measures approximately 3 inches wide by 3 1/2 inches long. Plan of Correction: The plastic area will be removed today.
Standard #: 22VAC40-185-350-E-4 Description: Based on observation, licensee did not ensure for children from three years to the age of eligibility to attend public school, five years by September 30 there was one staff member for every 10 children. Evidence: On March 27, 2019, at approximately 8:30 am, thirteen children were observed in the Ladders 1 room, (age range 4 to 5)with only one staff, staff #6. At approximately 8:44 am, a second staff entered the room, staff #4 and there were thirteen children with two staff. Plan of Correction: The center director will make sure the class is in ratio.
Standard #: 22VAC40-185-510-E Description: Based on observation and staff interviews, licensee did not ensure medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Evidence: On March 27, 2019, one medication, ProAir was observed and did not have the child's name, the dosage amount, and the time or times to be given. Plan of Correction: The center director will send it home with the parents and tell her to return it with the box today.
Standard #: 22VAC40-185-560-K Description: Based on observation, licensee did not ensure tables were sanitized before and after each use for feeding. Evidence: On March 27, 2019, the following classrooms were observed and the tables were not sanitized as required; 1) In the Ladders 1 room, children were observed prior to being served breakfast and they were reading books at the tables and then breakfast was served and the table was not sanitized prior to the children eating. 2) In the Bricks 2 room, children were observed prior to being served breakfast and they were playing with blocks at the table and and then breakfast was served and the table was not sanitized prior to the children eating. Plan of Correction: The center director will make sure the tables are sanitized.
Standard #: 22VAC40-185-560-N Description: Based on observation and staff interviews, the licensee did not ensure no child shall be allowed to drink or eat while walking around. Evidence: On March 27, 2019, in the Mirrors 11 room, child #3 was observed walking around drinking and center staff confirmed there was water in their water bottle. Plan of Correction: The center director will ensure that children are not walking and eating and drinking in the classroom.
Standard #: 22VAC40-185-570-H Description: Based on observation, licensee did not ensure prepared baby food not consumed during that feeding by an infant may be used by that same infant later in the same day, provided that the food is not served out of the baby jar and is dated and stored in the refrigerator; otherwise, it shall be discarded or returned to the parent at the end of the day. Evidence: On March 27, 2019, in the Mirrors iII room in the refrigerator one container with infant food was observed, center staff confirmed it was carrots and it was labeled 3-26-2019. One bottle labeled with a child's name and "form" was dated 3-25. Both containers were not sent home at the end of the day. Plan of Correction: The center director will make sure the food in the refrigerator is discarded today.
