Comments:
An unannounced monitoring inspection was conducted from 8:06 am-9:30. There were a total of 53 children with 5 staff members present. Children were observed arriving to the before school program, and engaged in group activities in the gym. A selection of staff trainings and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-60-A
Description: Based on children record review, physical and immunization records are not maintained in the child's record. Evidence: Child #1, enrolled 8/23/18 does have a copy of the physical and immunization record. Child #2, enrolled 8/2013 does not have a copy of the physical.

Plan of Correction: We will get copies from the school office.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children's medications, there is no written authorization from the child's physician and parent for medications on site. Evidence: Child # 3 and #4 have an inhaler medication on site without a written authorization form signed by the parent and physician.

Plan of Correction: We will contact the parent, the medication will be sent home with the parent.

Standard #: 22VAC40-185-510-G
Description: Based on a review of children's medications, the center has a medication on site that is not in accordance with the prescriber's instructions pertaining to dosage, frequency, and manner of administration. Evidence: Child # 5 medication authorization form dated 8/24/18 is to administer .3mg of the medication. The prescription label is to administer .15 mg of medication.

Plan of Correction: We will contact the parent to notify them of the physician error, an up to date medication authorization form will be obtained.
