An unannounced renewal inspection was conducted today from 10:30am-2:00pm. There were 17 children (4yrs-12yrs.) directly supervised by 4 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play in the gym, and eating lunch. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-160-C Description: Based on record review, the center did not obtain tuberculosis (TB) results at least every two years from the date of the last screening or testing. Evidence: Staff#1's record contains a TB test/screening that was last updated on 1/18/2017. Plan of Correction: We will schedule a TB test for the staff member.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center did not maintain complete children's records. Evidence: Child#4's record is missing the father's employer, and his work telephone number. Plan of Correction: We will talk to the vendor about making it a required field.
Standard #: 22VAC40-185-560-G Description: Based on observation, it was determined that lunch boxes brought from home by the children were not labeled with the current date. Evidence: Children were observed having lunch, 14 out of 17 lunch boxes were observed labeled with the child's name but without the current date. Plan of Correction: We will send a reminder to the parents.
Standard #: 63.2(17)-1720.1-A Description: Based on record review the center did not update a central registry check result for each staff member every five years following the last search of the central registry. Evidence: The records of Staff#2 contain a central registry search that was last updated on 5/7/2013. Plan of Correction: We will submit a new form on April 1st.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on staff record review, it was determined that the center did not complete fingerprint background checks for each staff member. Evidence: The record of staff#1 does not contain a fingerprint background check result. Plan of Correction: I believe they've been done, we just need to find the fieldprint email.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on staff record review, there is no documentation that Child Abuse and Neglect checks were initiated for employees that have resided outside the state of Virginia within the last 5 years. Evidence: Staff#1 has resided outside the state of Virginia within the last 5 years, their record did not have documentation that out-of-state child abuse and neglect results were obtained. Plan of Correction: We will submit the form to the appropriate agency.
