Comments:
An unannounced monitoring inspection was completed on 3/28/19. There were 73 children present. Five children's records in addition to four children's records with violations from the last inspection were reviewed. Five staff records were reviewed. No medications are administered at the center. The inspector arrived at the center at 2:30 pm and departed at 5:30 pm. The children were observed at arrival, during homework time, during free time in the game room, during a gardening project with community volunteers, and during snack. There was discussion with the front desk staff person about the enrollment form set up to have three emergency contacts, one of which does not have a space for an address. The name, address, and phone numbers are required for two people other than the parents who can be contacted in an emergency. There was discussion with the program director about the method of sanitizing the tables before eating.

Description: Based on observation, the center failed to ensure that hand washing was practiced as required. Evidence: 1. The children were observed during snack. None of the children washed their hands or used disposable wipes after eating.

Plan of Correction: The program director will have all children wash hands or use disposable wipes after eating.
