The licensing inspector conducted an unannounced monitoring inspection Tuesday, March 19, 2018 from 11:50am to 1:45pm to. The children were observed resting on cots during the inspection. Five children's records and two staff records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 02/04/2019 Last shelter in place drill: 11/05/2018 Total enrollment today: 18 Today, the following child to staff ratios were observed: Toddlers-5:1 Preschool (2 years old to 4 years old)-13:1 If you have any questions about this inspection, please contact your licensing inspector, Roshon Casey, at (804) 662-9760.

Standard #: 22VAC40-185-140-B Description: Based on a review of children's records on 03/19/2019, the center did not ensure that if a child, two years of age through five years of age has had a physical examination prior to attendance, it shall be within 12 months prior to attendance. EVIDENCE: The physical in the record of Child #4, who started at the center on 12/03/2018, is dated 02/10/2017. Plan of Correction: Going forward will look at where the doctor signed on the physical.
Standard #: 22VAC40-185-60-A Description: Based on a review of children's records on 03/19/2019, the center did not ensure that each child's record contained the required information. EVIDENCE: 1. The record of Child #4 did not contain an address for the second emergency contact listed. 2. The record of Child #5 did not contain documentation of child updates and confirmation of up-to-date information in the child's record. Plan of Correction: 1. Contacted parent to get information. 2.Will look at all files and see if they need updates today, Will get her parent to update today.
Standard #: 22VAC40-185-350-E-3 Description: Based on observation on 03/19/2018, the center did not ensure that for two-year-old children, the staff to child ratio is one staff member for every eight children. EVIDENCE: The staff to child ratio in the preschool class (children aged two to four years old) was one staff to 13 children. Plan of Correction: Had staff call out today. Already in process to hire additional staff tomorrow.
