- Technical Assistance:
The licensing inspector conducted a unannounced monitoring inspection Tuesday, March 26, 2019 from 10:20 am to 2:45 pm. The children were observed engaged in a variety of activities. The infants were observed sleeping in cribs and playing with toys on the floor with their teachers. Ten children?s records and Ten employee records were reviewed during this inspection. Medication is being administered, however none was present The center's first aid kit and emergency supplies were inspected. Last emergency drill: 02/06/2019 Last shelter-in-place drill: 02/19/2019 Total enrollment: 106 Today, the following child to staff ratios were observed: Infants I ? 15:4 Infants II-8:2 Toddlers ?19:4 Early Preschool (Twos) ? 26:4 Threes ?20:2 Pre-K ?18:2 School Age ?not present PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #: 22VAC40-185-130-A Description: Based on a review of children's records on 03/26/2019, the center did not ensure that documentation that two of the ten records reviewed contained documentation of immunizations before the child can attend the center. EVIDENCE: 1. The immunizations in the record of Child # 2, whose start date was 01/28/2019, are dated 02/27/2019. 2.
