Comments:
The licensing inspector conducted an unannounced monitoring inspection Wednesday, March 27, 2019 from 10:39am to 1:50pm. The children were observed playing in learning centers, playing outside, and eating lunch. The infants were observed playing on the floor and sleeping. All classrooms were inspected today. The facility?s mini buses and playgrounds were inspected today as well. Ten children?s records and ten employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 02/20/2019 Last shelter-in-place drill: 09/17/2018 Last fire inspection: 4/17/2018 Last health inspection: 11/30/2018 Total enrollment today: 113 Today, the following child to staff ratios were observed: Beach Babies (Infants 6weeks-12 months)-8:2 Beach Balls (Infants 8/9 months- 16 months) -4:1 Toddling Turtles (Toddlers)- 9:2 Tiny Bubbles (Toddlers)- 4:1 Sunny Sailboats (Twos)- 14:2 Jumping Jellyfish (Twos)-15:2 Silly Sandcrabs (Threes)- 10:1 Wacky Waves (Threes)- 20:2 Super Surfers (Pre-K)- 19:2 Lighthouse Room (Pre-K)- 20:2 Surf Shack (School Age)- not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-160-A
Description: Based on a review of staff records 03/27/2019, the center did not ensure that the documentation of a negative tuberculosis(tb) screening shall have been completed within 12 months prior to employing. EVIDENCE: The tb screening in the record of Staff #3, whose date of hire was 1/10/19, is dated 09/14/2016.

Plan of Correction: Will have TB within two weeks.

Standard #: 22VAC40-185-280-B
Description: Based on observation on 03/27/2019, the center did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. EVIDENCE: 1. A spray bottle of fluid labeled bleach h2o was observed on a shelf in the Super Surfer room. 2. A spray bottle of fluid labeled bleach h2o, a container of disinfectant wipes, and a canister of disinfectant spray was located in an unlocked closet in the Jellyfish room.

Plan of Correction: Will go over policies at the next staff meeting with all staff. Will get a health and safety person who will check and monitor during the day.

Description: Based on a review of children's records on 03/27/2019, the center did not ensure that they request annual updates to child record. EVIDENCE: The annual update in the record of Child #1 is dated 09/25/2017.

Plan of Correction: Will have parents update this afternoon.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of staff records on 03/27/2019, the center did not ensure that one of ten records reviewed contained a central registry finding within 30 days of employment. EVIDENCE: The central registry findings in the record of Staff #3, whose date of hire was 1/10/2019, are dated 09/14/2016.

Plan of Correction: Send off CPS request by the end of the week.
