Comments:
An unannounced monitoring inspection was conducted on Tuesday, March 19, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 9:30 am and concluded the inspection at 1:30 pm. The center?s director assisted the inspector throughout this inspection. The census for today?s inspection consisted of 45 children in the direct care of 8 staff and one administrator. Upon the inspector's arrival, the children were engaged in a variety of activities. Some of the children were observed playing in different centers, and interacting with peers and staff. The Preschool children were observed coloring the letter ?V? during a group activity. The Toddlers were observed during snack time. Diapering and hand-washing procedures were observed in the Infant room. The children in the Two?s classroom were observed during a teacher-led craft activity. They were making cocoons out of wooden sticks and yarn. Staff were observed having positive interactions with the children. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. The playground was inspected. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The children provide their own lunch and snack is provided by the center.

Five children's records and five staff?s records were reviewed during today's inspection.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on a review of children's records, observation, and interview on 03/19/2019, the center did not ensure to follow the center's written medication policies. Evidence: 1) An Epipen for Child #5 (DOE: 10/25/2016) was observed at the center. 2) During interview, a member of management stated that the center's medication policies were revised January 2019. The center's written medication policies indicate the center does not administer prescription or non-prescription medication.

Plan of Correction: Per the Center: "We do not administer medication, even emergency and will send a new notice home."

Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records on 03/19/2019, the center did not ensure that two records contained the required information. EVIDENCE: 1) The record for Child #1 (DOE: 10/15/2018) did not contain a complete address for the second emergency contact listed. The street name and house number were missing. 2) The record for Child #4 (DOE: 03/11/2019) did not contain a complete address for the second emergency contact listed. The house number was missing. Each child record shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Per the Center: I will be more careful to make sure streets and numbers are written.

Standard #: 22VAC40-185-280-B
Description: Based on observations at the center on 03/19/2019, the center did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) The licensing inspector observed bottles of disinfectant spray and two bottles of bleach on top of a bookshelf in the Threes classroom. The bottles were out of reach of the children, but were not kept locked using a safe locking method that prevents access by children. 2) The licensing inspector observed a bottle of air freshener on top of the toilet in the Threes bathroom. 3) The licensing inspector observed a bottle of bleach in an unlocked cabinet under the sink in the Twos classroom. 4) The licensing inspector observed two bottles of disinfectant spray in a bookshelf in the Twos hallway.

Plan of Correction: The cleaning materials were removed from the classrooms and placed in a locked cabinet while the inspector was on site. Per the Center: "Will have staff utilize licensing checklists daily."

Standard #: 22VAC40-185-290-3
Description: Based on observations at the center on 03/19/2019, the center did not ensure all electrical outlets have protective covers. Evidence: 1) The licensing inspector observed two electrical outlets without a protective cover in the bathroom used by the Twos class.

Plan of Correction: The electrical outlets were covered while the inspector was on site. Per the Center: "Will remind staff and again utilize licensing checklist.

Standard #: 22VAC40-185-320-B
Description: Based on observation at the center on 03/19/2019, the center did not ensure the restroom used by the children in care was equipped with disposable towels or an air dryer within reach of children. Evidence: 1) The licensing inspector observed the Twos restroom did not have paper towels available for the children in care.

Plan of Correction: Corrected on site. Per the Center: "I will remind teachers to keep all rooms stocked at all times."

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of five staff records on 03/19/2019, the center did not obtain the results of a national fingerprint-based criminal history record check for two staff members within the required timeframes. Evidence: 1) The record for Staff #2, hired on 03/29/2017, contained the results of a national fingerprint-based criminal history record check dated 11/13/2018. 2) The record for Staff #5, hired on 08/10/2018, contained the results of a national fingerprint-based criminal history record check dated 08/27/2018. As of 01/22/2018, a statement of eligibility based on a national fingerprint-based criminal history record check is required to be obtained prior to employment. For staff employed prior to 01/22/2018, a statement of eligibility based on a national fingerprint-based criminal history check was required to be obtained prior to 09/30/2018.

Plan of Correction: Per the Center: "No employee starts orientation without fingerprint background checks come back. I was unaware of the rules due to confusion on my part, but fully understand now."
