Discussed the following with the administrators: - Resources to complete (and follow up with) the search of the central registry in Virginia and other states - Location of the Inspection Findings report - Sample forms available on dss.virginia.gov
An unannounced follow up inspection from the license issuance was completed today. The physical plant, 5 staff files, 5 children's files, annual fire and health inspection, staff training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 1:30-3:35pm. There were 3 children under the supervision of 3 staff. During the inspection, the children were observed resting during naptime, cleaning up and preparing to leave for the day. The children bring their lunches and snacks from home each day they receive care. There are no children with any medication needs during program hours at this time. The interactions between the children and staff were seen to be positive and constructive in nature. Should you have any questions, please contact me at email@example.com.
Standard #: 22VAC40-185-240-D-5 Description: Based on file review and interview, there is no staff on site today who has obtained within the last three years instruction in performing the daily health observation (DHO) of children. Evidence 1. The Director stated that no one currently has attended this training, but they have scheduled a DHO training to attend. Plan of Correction: All of the staff ate scheduled to attend this training.
Standard #: 63.2(17)-1721.1-B-3 Description: Based on file review, 1 staff record did not have a search of the central registry (SCR) completed for a state in which the individual has resided in the preceding five years. Evidence 1. Staff #2 has documentation of having lived in another state in 2015, but the SCR was not completed for this state. Plan of Correction: The administrator worked on having this submitted for completion during the inspection.
