Comments:
An unannounced monitoring inspection was conducted on Tuesday, March 26, 2019 to determine the center?s compliance with licensing standards. The inspector arrived at 2:55 pm and concluded the inspection at 6:00 pm. The center?s director assisted the inspector throughout this inspection. The census for today?s inspection consisted of 35 children in the direct care of 4 staff and one administrator. Upon the inspector's arrival, children were engaged in various activities. The children were observed playing UNO with a staff member and later observed doing homework and reading books. Two other groups of children were observed during snack time and later playing in different centers in their designated area of the classroom. The children were observed taking bathroom breaks and later going outside. Some of the children were picked up while the inspector was on site. Staff interacted with the children in a positive manner. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected and found complete. Five children's records and five staff records were reviewed during today's inspection. Emergency medications and authorizations were reviewed. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records on 03/26/2019, the center did not ensure that two records contained the required information. EVIDENCE: 1) The record for Child #2 (DOE: 01/22/2019) did not contain a second emergency contact. 2) The record for Child #4 (DOE: 01/07/2019) did not contain the address for the second emergency contact listed. Each child record shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Per the Center: Will correct in the morning. In the future, the director will monitor all paperwork to ensure it is completed upon enrollment.

Standard #: 22VAC40-185-70-A
Description: Based on a review of five staff records on 03/26/2019, the center did not obtain two references as to the character, reputation and competency for one staff member before employment. Evidence: 1) The record for Staff #5, hired on 07/09/2018, contained one reference check that was dated 07/10/2018 and a second reference check that was dated 07/11/2018. Each staff record shall contain documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.

Plan of Correction: Per the Center: In the future, the center will make sure all reference checks are obtained prior to employment.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of five staff records on 03/26/2019, the center did not obtain the results of a fingerprint-based criminal history record check for one staff within the required timeframe. EVIDENCE: 1) The record for Staff #5, hired on 07/09/2018, contained the results of a fingerprint-based criminal history record check dated 07/25/2018. As of 01/22/2018, a statement of eligibility based on a national fingerprint-based criminal history record check is required to be obtained prior to employment.

Plan of Correction: Per the Center: The center will make sure all fingerprints are completed prior to hire.
