Comments:
An unannounced Monitoring Inspection, and a Self-Reported incident were conducted today from 8:40am to 12:00pm. On 12/27/18 the facility self-report an incident where a child was left on the playground, unsupervised, for approximately 4 minutes. Based on a preponderance of evidence, the self-reported incident is deemed VALID. There were 57 children ages 0-5 years, directly supervised by 13 staff. The physical plant, 9 staff records, 5 children?s records, 1 child's medication and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, individual activities, circle time and pictures for picture day. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on review, staff did not ensure that they were following the facility's policies and procedures, required by these standards, or playground safety and supervision; requiring staff to conduct name to face head counts prior to bringing children inside from the playground. Evidence: Identified through staff interviews, and reviewed in staffs' written statements, that Staff 1 did not conduct a name to face head count after leaving the playground and going into the classroom, which lead to one child being left behind, and unsupervised on the playground for approximately 4 minutes.

Plan of Correction: Staff 1 counted children as they came in from the playground but did not do name to face. Staff 1 was placed immediately on administrative leave and is no longer employed by Minnieland.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: Observed gaps on the climbing equipment with the blue slides, between the platform and the slides, causing an entanglement hazard.

Plan of Correction: Playground is currently under renovation and is not in use at this time and has not been in use during renovation process. This was corrected during inspection.

Description: Based on observation, the facility did not ensure that electrical outlets have protective covers. Evidence: 1. Observed in Room 2, 3 uncovered electrical outlets on a surge protector. 2. Observed in Room 11, 8 uncovered electrical outlets on two surge protectors near the computer.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-340-F
Description: Based on staff interviews and written statements, the facility did not ensure that children under 10 years of age are always within actual sight and sound supervision of staff. Evidence: Staff 1 was on the playground with 14 children, and without conducting name to face checks, escorted the children into the classroom, leaving Child 1 alone on the playground for approximately 4 minutes. Staff 2 saw Child 1 on the playground, coming into the classroom and notified the assistant director and spoke with the teacher. Staff 1 stated in a written statement that they did not know that Child 1 was on the playground until they came inside the classroom.

Plan of Correction: Upon interviewing school age child, the child attempted to hiding from her teacher. She was spotted by another teacher moving from one hidden spot to the next. Staff was immediately placed on administrative leave and is no longer employed with Minnieland. All staff have been retrained on name to face accountability.
