Comments:
An unannounced renewal inspection was conducted today from approximately 9am-11:30am. There were 70 children directly supervised by 18 staff. A sample of 7 children?s records, 5 staff records and 3 minor injury logs were reviewed. The children were observed learning about body parts, singing, playing on the playground, reading books, and engaged with teachers. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were incomplete. Evidence: 2 of 7 Children's records reviewed were missing required information. 1) Child A was missing 2 of 2 required emergency contacts. 2) Child B was missing the address of an emergency contact.

Plan of Correction: I will ask her to fill that out.

Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records were incomplete. Evidence: 1 of 5 staff records reviewed was missing required information. 1) The record of Staff A was missing a copy of their diploma or transcript.

Plan of Correction: I will ask her to bring it in.

Standard #: 22VAC40-185-340-F
Description: Based on observation and interview with staff, children under 10 years of age were not always within actual sight and sound supervision of staff. Evidence: 1) One Child from the "Green" room and two children from the "Orange" room were witnessed leaving the room unsupervised going to the bathroom and re-entering the room. 2) Staff B from the "Green" room stated the child was 4 years old. 3) Staff C from the "Orange" room stated the children in the room were 4 and 5 years old.

Plan of Correction: Staff will bring groups of children to the bathroom to maintain ratios.
