22VAC40-665 STAFF QUALIFICATIONS & TRAINING
A Subsidy Health and Safety inspection was conducted from 2:15pm-3:30pm to review supplemental health and safety requirements. At the time of entrance, 64 children were in care of 8 staff members present. Children were observed during arrival to the after school program, board games, homework, handwashing, snack time and outdoor play time. A selection of 4 staff records, 10 childrens records, medications and other documents were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-520-B-11 Description: Based on records reviewed, the record for one child with a diagnosed food allergy did not include steps to be taken in the event of a suspected allergic reaction. Evidence: A physical examination form for Child #1 documented a food allergy to peanuts. There was no documentation of an allergy response plan available during the inspection. Plan of Correction: The parent documented that the child does not have an allergy. We can request updated documentation from the parent to verify whether the child has any allergies; however, it is anticipated that the child's care will soon be discontinued [as they are aging out of the program].
