Technical Assistance:
Consultation was given on lead teacher qualifications and training requirements within a month of being promoted as well as infant crib spacing when all cribs are occupied.

Comments:
An unannounced monitoring visit was conducted from 11:00-1:30. All rooms were in ratio with direct care staff. Observed children transitioning from outside, infant care, diapering, lunch (mac and cheese, mixed vegetables, apples and milk), emergency preparation, trainings, medications, a sample of children records and newly hired staff. Description: Based on record review, 1 of 5 children's records does not have an additional immunization record for a child under two years of age. Evidence: Child #3's immunization record was last updated 8/2018.

Standard #: 22VAC40-185-270-A
Description: Based on observation, Areas and equipment of the center, inside and outside, are not maintained in a clean, safe and operable condition. Evidence: 1. Paint is peeling in the following classrooms:Toddler 1, Toddler 2, Toddler 3, Pre-School 2, Infant 3 and Twos-3. 2. The wallboard by the hall storage closet is cracked. 3. The cabinet veneer is peeling on 3 cabinets along the bottom in Toddler 1 4. The red couch in Toddler 3 has split and worn plastic covering on the arm rest.

Evidence: Classroom Two's-3 has not had a lead qualified staff since the end of December 2018 Classroom Infant-1 has not had a lead qualified staff since mid November 2018.

Description: Based on observation, food bought from home was not clearly labeled to identify the owner. Evidence: Two baskets for children in the Toddler-2 classroom had food pouches/dried fruit snack bags that were not labeled and dated to clearly identify the owner.
