Technical Assistance:
Technical assistance was provided regarding raking the mulch under the swings and at the bottom of the slides. There isn't a fire drill documented for March, but still have time to do one today.

Comments:
An unannounced monitoring inspection was conducted today from 9:45am-1:15pm. There were 57 children (infants-5yrs.) supervised by 11 direct care staff. The physical plant, programming, staff records, children?s records, medications, evacuation drills, and policies were inspected. Children were observed participating in group play, reading books, playing outside, practicing their spelling, and building houses. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Description: Based on review of staff records, 1 staff member does not have documentation of an updated tuberculosis (TB) screening every 2 years. Evidence: Staff #2 has documentation of an expired TB screening on record, 1/16/17

Plan of Correction: We make sure she has her TB screening done and going forward we will review our spreadsheet monthly.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, 1 record is not complete. Evidence: Staff #1 does not have documentation of their education.

Plan of Correction: We will get a copy and put it in her file. Going forward we will make sure to have a copy when they are hired.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were located in an unlocked cabinet, making them accessible to children. Evidence: A hazardous disinfectant spray was located in an unlocked drawer in the hallway to the outside.

Plan of Correction: We will move the disinfectant.

Description: Based on review of 6 children's records, 5 are missing annual parental updates. Evidence: There is not an annual update of emergency contacts for children #2,3,4,5 & 6. Last updates fro 9/17.

Plan of Correction: We will review the records and put a system in place to get an update yearly.

Description: Based on review of medications, not all medications have authorizations. Evidence: Child #1 has a medication authorization that does not authorize Winwood VIII to administer the medication.

Plan of Correction: We will have the parents get a new authorization form.

Description: Based on observation, food brought from home was not labeled with child's name or date. Evidence: Lunch boxes located in the math & science room and language arts room, containing food brought from home did not have any identification inside or outside the box.

Plan of Correction: We will check when they come in the morning to make sure they have a name and date.

Description: Based on observation, not all bottles in the refrigerator are labeled with child's name and date. Evidence: Located in the Infant refrigerator were bottles without a name, and some without a date.

Plan of Correction: Corrected during the inspection. We will make sure to check them before the parents put in the refrigerator.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of staff records, 1 record is missing an out of state background check. Evidence: Staff # 1 has lived out of state within the past 5 years and does not have documentation of an out of state background check being completed.

Plan of Correction: We will send it out and email you the results when we receive them.
