Comments:
An unannounced monitoring inspection was conducted today with the director. Today is teacher's workday and two sites are combined which resulted in 6 staff on site with 36 children. The children were transitioning to the gym for fitness time and then returned to the cafeteria for lunch. After lunch, the teachers and children went outside for playtime. Medications for seven children were reviewed as well as 8 children and 4 staff files. Arrival time: 11:15 am Departure time: 1:05 pm

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of eight children's files, it was determined that three children did not have physicals on file within 30 days of enrollment. Evidence: Child C, D and E enrolled on 8/27/18 and they do not have physicals on file.

Plan of Correction: I have previously notified these parents of the need for physicals. I will remind the parents and give them a specific date to get me the physicals.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medications for seven children , it was determined that two children did not have authorization for medication on-site. Evidence: Child A and B have Auvi-Q on-site but did not have authorization for this medication from the parent and/or physician.

Plan of Correction: We had authorization for Epipen Jr., for both children and we will get the proper authorization and place in the file.
