Comments:
An unannounced inspection was conducted today, February 20, 2019 in response to a complaint received by The Department of Social Service, Children's Licensing on February 19, 2019 concerning diapering procedures and staff supervision . The inspector was on site from 4:00 pm am until 5:15 pm. Staff were interviewed and a classroom observation was conducted. A staff record and one child's record were reviewed. Based on the information gathered during the inspection, the preponderance of evidence does not support the allegation, therefore the complaint is not valid. Though not part of the original complaint there were violation found during the inspection regarding staff records, background checks, hand washing and physical plant. Standard #: 22VAC40-185-70-A
Complaint related: No
Description: Based on review of staff records, the facility did not obtain all required information for each staff record. Evidence: Staff #1, hired 10/24/2018, did not have documentation that two or more references as to character and reputation as well as competency were checked prior to employment.

Plan of Correction: Per the director: I will complete reference checks at the time of hire.

Complaint related: No
Description: Based on observation, the facility did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. A spray bottle of window cleaner was observed unlocked under the changing table in the two year old classroom. The latch of the changing table cabinet was broken. 2. Four canisters of disinfectant spray and sanitizer were observed unlocked on the shelf, approximately five feet from the floor, in the two year old classroom.

Plan of Correction: The director removed the cleaning supplies. Per the director: I will have a new latch put on the cabinet and have staff keep all cleaning items in the locked cabinet.

Complaint related: No
Description: Based on observation, the facility did not ensure electrical outlets had protective covers installed. Evidence: Eight electrical outlets located behind a wooden box attached to the wall in the two year old classroom did not have protective covers installed. The wooden box was approximately three feet from the floor and the latch on the doors of the box were inoperable make the outlets accessible to children in care.

Plan of Correction: Per the director: i will put covers on all the outlets.

Complaint related: No
Description: Based on observation, the facility did not ensure staff washed their hands with soap and running water before and after helping a child use the toilet or a diaper change. Evidence: 1. After changing a diaper of a child in the two year old classroom, staff #1 did not wash her hands nor the hands of the child that was diapered. 2. The inspector asked staff #1 to wash her hands and the hands of the child.

Plan of Correction: Per the director: We will review hand washing procedures with staff.

Standard #: 22VAC40-191-60-C-2
Complaint related: No
Description: Based on review of staff records, the facility did not obtain a central registry finding within 30 days of employment. Evidence: The record of staff #1, hired 10/24/2018, had documentation of a central registry finding dated 11/29/2018, exceeding 30 days from hire.

Plan of Correction: Per the director: I will have the background requested within the first seven days of hire and call to check if they are not returned by 30 days.
