While conducting an unannounced on-site complaint inspection on 3/1/19 regarding a child being injured at the center, a violation was noted in the subsidy regulations. Please see attached violation notice.

Standard #: 22VAC40-665-550-2 Description: Based on interview, the vendor failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional. Evidence: 1. On 2/26/19, a child was injured at the center and taken for outside medical treatment. The vendor failed to inform the department of the injury within the required time frame. 2. The center director confirmed that the department was not notified that a child was injured at the center and taken for outside medical treatment. Plan of Correction: Going forward, the vendor will ensure that all serious injuries requiring outside medical treatment are reported to the department within the required time frame.
