An unannounced monitoring inspection was conducted today. At the start of this inspection there were 38 children in care, with a total of 5 staff members assigned to their direct supervision. Children's records, staff records, staff training records, background checks, health records, the physical plant, and emergency preparedness items were inspected today. The children were observed playing on the playground, playing with toys, finishing lunch from home, and singing songs. Dismissal of children from the morning program, and the arrival of children for the afternoon program was also observed. Pleasant and friendly interactions between children and staff were observed in all classrooms. This inspection was completed between approximately 11:45am and 1:10pm. Standard #: 22VAC40-185-40-G Description: Based on a review of children's administrative records, it was determined that one record is not complete. Evidence: Child record C does not include documentation showing that a staff person has viewed the child's original proof of identification. Plan of Correction: The parent arrived to pick up the child during this inspection. The parent was asked to bring the child's ID tomorrow.

Evidence: Child record C does not include the address for each emergency contact person. Plan of Correction: The parent arrived to pick up the child during this inspection. The parent was asked to bring the information tomorrow.
Standard #: 22VAC40-185-270-A Description: Based on observations, it was determined that one area has not been maintained in a clean condition as required. Evidence: In the children's bathroom, there are areas of rust on the partition walls separating the toilet areas. Plan of Correction: I will ask maintenance to repair the walls and remove the rust.
