Technical Assistance:
**Discussed with the site director that several children on two occasions were observed teasing a specific child in care, and both times staff were nearby and only observed and did not intervene. Spoke to the site director about reviewing recent training certificates on preventing bullying and the need to work with staff to ensure that they are intervening sooner.

Comments:
An unannounced Monitoring Inspection was conducted today from 7:27am to 8:45am. There were 24 school-agers directly supervised by 5 staff. The physical plant, 8 staff records, 8 children?s records, 3 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed arriving to care, playing group games, coloring, singing, taking morning naps, playing basketball and eating breakfast. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-60-A
Description: Based on review, the facility did not ensure that children's records were maintained with the required information. Evidence: Reviewed 8 children's records and found that 1, Child 5 did not have documentation of a parent work telephone number.

Plan of Correction: Will obtain by the end of the week.

Standard #: 22VAC40-185-210-A
Description: Based on review, the facility did not have documentation of two staff members that were considered program leaders, with program leader qualifications. Evidence: Reviewed 8 staff files and found that Staff 2 and Staff 3 were program leaders, but did not have documentation in their records of program leader qualifications.

Plan of Correction: Will obtain from main office.

Standard #: 22VAC40-185-540-E
Description: Based on observation, the facility did not have a working radio or flashlight on site. Evidence: Staff had a radio and flashlight on site, but could not get them to work during the inspection visit.

Plan of Correction: Will obtain batteries today.

Standard #: 22VAC40-185-560-N
Description: Based on observation, the facility did not ensure that children were not eating or drinking while walking around. Evidence: 1. Observed one child during breakfast walking from the trash can to the table, then to their backpack while eating. 2. Observed one child getting up from their table, get on the floor to do push-ups and then back up to the table while eating.

Plan of Correction: Corrected at the time of the visit.
