Comments:
A Subsidy Health and Safety inspection was conducted today at 7:27am to 8:55am to review supplemental health and safety requirements. There were 24 children under the supervision of 5 staff at the time of arrival. Children were observed playing group games, playing basketball, singing and eating breakfast.. There were 3 medications on site today. There was a sufficient supply of books, toys, and materials that were developmentally appropriate for the children in care. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-540-A
Description: Based on review, the facility did not ensure that staff obtain a TB screening at the time of employment or prior to coming into contact with children. Evidence: Staff 5 did not have documentation of TB screening. The staff member had gone in for a test, but did not return to get documentation of the results of their test.

Plan of Correction: Will get them to get it completed.
