An unannounced monitoring inspection was conducted today from 10:50am-2:00pm. There were 35 children (2-6yrs.) supervised by 6 direct care staff. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed engaging in various Montessori activities, including leaf prints, land and water forms, sorting, and writing. Toileting and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Description: Based on record review, 1 of 4 children's records did not contain documentation of all of the required information. Evidence: Child #3's (start date: 1/2019) did not include documentation of the mother's work telephone number.

Plan of Correction: We will get it right away.

Standard #: 22VAC40-185-240-C
Description: Based on record review, 1 of 3 staff records did not contain documentation of 16 hours of staff development and training annually, as required. Evidence: Staff A's (date of hire: 9/2013) record did not contained documentation of 14 out of the required 16 hours of staff development and training.

Plan of Correction: We will make sure all staff receive all 16 training hours this year.

Description: Based on a review of medications, 1 of 3 children's medications did not comply with methods to prevent use of outdated medication. Evidence: Child #1's (start date: 1/8/2018) Epi-Pen Jr. expired 1/2018.

Plan of Correction: Medication was sent home today.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medications, 1 of 3 children's medications was not picked up by the parent within 14 days of the authorization expiration, nor was disposed of by the center. Evidence: Child #1's (start date: 1/8/2018) medication authorization form expired 12/30/2018, and the medication was on site during the time of inspection.

Plan of Correction: We will have the parents return an updated authorization form the next time the child arrives at school.

Standard #: 22VAC40-185-550-D
Description: Based on record review, the center did not maintain documentation of two shelter-in-place practice drills per year. Evidence: There was documentation of one shelter-in-place drill conducted for 2018.

Plan of Correction: We will do one today.
