Technical Assistance:
Discussion of the subsidy regulations and how they apply to the Child Day Center (CDC) to include the additional changes that went into effect October 17, 2018. A copy of the email dated 10/16/2018 that listed the substantive changes and group size was given to the center.

Comments:
A Subsidy Health and Safety (SHSI) inspection was conducted at the Goddard School located in Leesburg, Virginia. This inspection was conducted on 04/05/2019, between the hours of 9 am through 3:30 pm to review supplemental health and safety requirements. An Unannounced Monitoring Inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that were not covered in licensing standards were reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and were identified on the violation notice. If you should have any questions, please call or e-mail. Violations:
Standard #: 22VAC40-665-580-B
Description: Based on review, 3 of 10 staff files reviewed indicated that within the first 90 days of subsidy vendor approval all staff who work directly with children did not complete the Virginia Pre-service Training for Child Care Staff. Evidence - 1. On the date of inspection (04/05/2019), the certification for the obtained 10 hours of Virginia Pre-service Training for Child Care Staff was unavailable for review for: Staff #4, Staff #5, and Staff #8. 2. This was confirmed on the date of inspection by the Center Director.

Plan of Correction: This training shall be administered for all applicable staff. Once obtained this certification shall be placed in each applicable file for future review.

Standard #: 22VAC40-665-650-E
Description: Based on interview, the center does not have a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence- 1. The Center Director stated (during an interview conducted on the date of inspection 04/05/2019) that the email dated 10/16/2018 that stated these substantive changes to the subsidy program were not received. 2. It was also stated that these procedures were not currently in place.

Plan of Correction: Written policy and procedures addressing this issue shall be developed, implemented, and placed in the parent handbook.

Description: Based on interview, the center does not have procedures in place to ensure that essential functions are maintained during an emergency. Evidence- 1. The Center Director stated (during an interview conducted on the date of inspection 04/05/2019) that the email dated 10/16/2018 that stated these substantive changes to the subsidy program were not received. 2. It was also stated that these procedures were not currently in place.

Plan of Correction: Written procedures addressing this issue shall be developed, implemented, and added to the center's Emergency Preparedness Plan.

Standard #: 22VAC40-665-780-A-3
Description: Based on review, lockdown procedures were not practiced at least annually. Evidence - 1. On the date of inspection (04/05/2019), the last documented lockdown drill conducted was dated - 02/22/2018. 2. This information was confirmed by the Center Director.

Plan of Correction: A lockdown drill shall be conducted immediately and will continue to be conducted on an annual basis (or sooner as needed).
