Discussed with center supervisor to implement a system to ensure that ratios are maintained when children from 5-8 years are present, even during transitions; and to discuss this with all staff that works in the center.
- Comments:

An unannounced monitoring inspections was conducted from 9:00am to 10:30am. There were 44 children present with 5 staff members. The children were observed eating breakfast, playing with toys, coloring, going to the gym and preparing for a field trip. 5 children's medications were reviewed. 5 children and 3 staff records were reviewed. Standard #: 22VAC40-185-130-A Description: Based on record review, the center did not obtain immunization records for children enrolled before the first day of enrollment. Evidence: 1. Child # 1's first day of enrollment was 9/8/15. There was no documentation of immunization records in Child # 1's file. 2. Child # 2's first day of enrollment was September 2018. There was no documentation of immunization records in Child # 2's file. Plan of Correction: I will request copies from both children's parents.
Standard #: 22VAC40-185-140-A Description: Based on record review, the center did not obtain a copy of the physical exam for children enrolled within 30 days of enrollment. Evidence: Child # 2's file did not have any documentation of a physical exam and it was not available for review. Plan of Correction: I will ask the parent to bring in a copy of the physical exam.
