An unannounced complaint inspection was conducted on April 1, 2019 from approximately 7:45 A.M.-11:45 A.M. in regards to a concern reported to the Valley Licensing Office on 03/21/2019 and on 03/28/2019. Six staff records and four children records were reviewed, seven staff members were interviewed, and four ready, set, go assessment forms were reviewed. The information gathered during the investigation does not support the allegation, so the allegation is determined to be "not valid". There were two violations cited not related to the allegation. Standard #: 22VAC40-185-160-A Complaint related: No Description: Based on review of staff files, the center failed to ensure that a negative tuberculosis screening was completed within 21 days of employment. Evidence: 1. Staff #1 tuberculosis screening was dated 12/6/2018. Date of hire was 11/09/2018. 2. Staff verified the date of the tuberculosis screening and the date of hire for Staff #1. Plan of Correction: In the future we will track an employee's start date to ensure the tuberculosis screening is completed in the required time frame.
Standard #: 22VAC40-185-500-A Complaint related: No Description: Based on observation, the center did not ensure that staff wash their hands with soap and running water after any contact with body fluids, and before feeding or helping children with feeding. Evidence: In the Two Year Old Classroom Staff #6 was observed assisting a child wipe their nose. After Staff #6 wiped the child's nose she did not wash her hands. Staff #6 went over to the counter and started assisting Staff #1 with serving breakfast. She picked up two sippy cups and moved them to the table. Plan of Correction: Administration will retrain all staff on hand washing requirements and do periodic checks in the classroom to ensure that hand washing procedures are being followed.
