Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 04/01/19. Interviews were conducted with staff, and inspector observation was conducted on 04/02/19 relating to allegations that ratios were not being maintained as required. The evidence gathered during the investigation supports allegations, so the complaint is determined to be valid. Please contact me if you need further assistance. Thank you.

Complaint related: No
Description: Based on discussions with staff and review of staff files the facility failed to maintain compliance with this standard as required relating to maintaining a completed file for each staff. Evidence: Staff #1-#5, all newly hired and actively working, did not have a completed file as required relating to proof of education, orientation, and references.

Plan of Correction: HR reviewing with RD and PD that all new staff hired and working must have completed file as required relating to proof of education, orientation, and references.

Description: Based on observation the facility failed to maintain compliance with this standard as required relating to supervision of the children. Evidence: The LI observed a 4 year old child in the main classroom area alone on 04/02/2019, wandering around. The actual time frame of how long the child was unsupervised was not able to be determined. Another staff member was made aware , by the inspector, of the child being alone and promptly assisted him into the classroom where he belonged. All the doors to the other adjacent classrooms were closed, and there was no staff supervision of this child as required.

Plan of Correction: All staff will receive additional training to review sight and sound supervision policies and procedures.

Description: Based on observation and discussions with the staff the facility failed to maintain compliance with this standard as required related to ratios of mixed aged groupings. Evidence: The LI witnessed on 04/02/2019, at 7:55am, one staff member with 20 children aged 4 years-10 years. The eleventh child arrived at 7:45am, placing the classroom out of the required ratio of 1 staff to 10 children. The room was out of ratio from 7:45am until 8:20am, when the children went to public pre-school/school. Interviews with staff confirmed that the following ratio issues also occurred: On 03/29/2019 there were 2 staff in the yellow room, ages 4 months-16 months, with 11 children from 7:45am-8:15am. The required ratio was 1 staff to 4 children. On 03/29/2019 there was 1 staff in the green room, ages 16 months-2 years, with 10 children from 7:45am-9am. The required ratio was 1 staff to 5 children.

Plan of Correction: The RD, PD is working with HR to ensure that they have a plan in place to hire staff to meet the needs of the center so that they are in ratio. They are in the process of hiring subs in the area to fill in when staff are out or while looking for staff.

Description: Based on discussions with administrative staff and review of staff files the facility failed to maintain compliance with this standard as required relating to Central Registry Clearances. Evidence: There were 5 new staff actively working alone with the children without having a completed Central Registry Clearance as required. The staff had been working prior to this complaint inspection as well. Staff #1 hire date 03/20/19 Staff #2 hire date 03/18/19 Staff #3 hire date 03/25/19 Staff #4 hire date 03/27/19 Staff #5 hire date 04/01/19

Plan of Correction: This has been reviewed with RD and PD that new staff cannot be left alone without having a completed Central Registry Clearance as required.
