The licensing inspectors conducted an unannounced renewal inspection on Tuesday, April 9, 2019 from 10:05 AM until 12:20 PM. The following ratios were observed with a total of 33 children present during the inspection:
Toddlers - 5 children with 1 staff
Twos & Threes Room - 10 children with 2 staff
Threes & Fours Room - 10 children with 1 staff
Fours & Fives Room - 8 children with 1 staff
The children and staff were engaged in a variety of activities while the licensing inspectors were on the premises. Children were observed playing on the floor with textured balls, trains, bristle blocks and a doll house. Children were also observed participating in Stretch-N-Grow, singing and practicing writing the letter "V" with bingo markers. Later, children were observed taking bathroom breaks, washing hands and eating lunch. All areas of the center including classrooms, hallways, kitchen, bathrooms and outdoor play area were inspected. The center was equipped with toys and supplies and items were available to the children.
The following information was reviewed: injury reports, injury prevention plan, daily attendance, practiced emergency drill logs, emergency preparedness plan, liability insurance policy, parent agreements, staff training information, required postings and menu. The center?s first aid kit and emergency supplies were inspected. A member of management stated medication is not administered and transportation is not provided at this time. Five children's, five staff and one board officer?s records were reviewed.

Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: During the inspection, the licensing inspectors observed aerosol cans of air freshener in an unlocked cabinet over the sink in the Three's and Four's bathroom. Plan of Correction: The aerosol cans were moved to a locked cabinet during the inspection.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on review of six records, one board officer member did not obtain satisfactory results of the fingerprint-based national criminal background check by 09/30/2018. Evidence: During the inspection, the licensing inspectors reviewed background checks for Board Officer #1. The record for Board Officer #1contained satisfactory results of the fingerprint-based national criminal background check dated 03/25/2019. Board Officer #1 has been with the center since 1999. Results of fingerprint-based national background checks were required to be obtained by 09/30/2018. Plan of Correction: The Director stated since Board Officer #1 is not in the center or working with children it did not occur to her that Board Officer #1 needed a national background check.
