Comments:
An unannounced monitoring inspection was conducted today from 10:00am-11:15am. There were 23 children directly supervised by 4 staff. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in sharing items with the letter "K" and returning from a field trip to the adjoining farm. Hand-washing procedures were also observed. Staff files are reviewed annually as they are kept off site at a central location. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, 3 of 5 children's records were incomplete. Evidence: Child 1, 3, and 5's records did not contain evidence that proof of identity was reviewed.

Plan of Correction: We will ask the parents to provide the information.
