An unannounced renewal inspection was conducted today from 7:10am-8:10am. At the beginning of this inspection there were 6 children directly supervised by 2 staff. The physical plant, 4 staff training records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in block building, coloring and playing with personal electronic devices. The center was preparing for a field trip to the movie theater later in the morning. l There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. If you have any questions regarding this inspection, please contact the Licensing Inspector, Kara Wright at email@example.com or 703-537-6241.
Standard #: 22VAC40-185-50-A Description: Based on observation, children's records were not treated confidentially. Evidence: The "Supplemental Page" from the previous licensing inspection was posted in the facility. The page contains information about the confidential data in children's records. Plan of Correction: We will take it down today.
Standard #: 22VAC40-185-60-A Description: Based on record review, 2 of 5 children's records reviewed were incomplete. Evidence: 1. Child 1's record did not contain the address of one of the two listed emergency contacts. 2. Child 5's record did not contain the name, address, phone number or employment information for the child's father. On the date of inspection there was no custodial paperwork in the record to confirm if there is only one legal guardian. Plan of Correction: We will collaborate with the parents to complete the missing information.
Standard #: 22VAC40-185-510-C Description: Based on medication review, the center did not have procedures in place to prevent administration of outdated medication. Evidence: Child 1 has two emergency medications present at the center which expired November 2018. Plan of Correction: We will ask the parents for updated medication.
