Technical Assistance:
Discussed with site supervisor that parental information must be included on children's forms or custodial paperwork present if a father is listed on the birth certificate.

Comments:
An unannounced renewal inspection was conducted today from 3:45pm-4:50pm. There were 13 children directly supervised by 3 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outside, doing homework and working on projects at the table. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid. Staff-to-child ratios were in compliance. The center was clean and organized. No violations were cited during today's inspection.
