An unannounced monitoring inspection was conducted today beginning at 7:45am and ending at 8:55am. There were 16 children present with 2 staff. Children were observed reading books, participating in arts and crafts activities, helping make quesadillas for breakfast, and other organized activities. 4 children's records, 2 staff records, emergency drills and procedures, the first aid kit and required postings were reviewed. There were no medications on site today. Staff records are reviewed annually in the administrative office, however, MAT (medication administration training) and first aid/CPR certificates were available for staff on site and all staff are current. Standard #: 22VAC40-185-140-A Description: Based on record review, 1 of 4 children's records reviewed did not contain documentation of a physical examination being obtained within 30 days of the child's first day of attendance. Evidence: 1. Child #1's (Date of enrollment 6/29/2015) record does not contain a physical examination. Plan of Correction: I will go to the school and get a copy.
Standard #: 22VAC40-185-60-A Description: Based on record review, 1 of 4 children's records reviewed did not contain two complete emergency contacts to include name, address, and phone number. Evidence: 1. Child #4's (Date of enrollment 9/2/2014) record did not have 1 of 2 emergency contacts. Plan of Correction: I will ask the parents to add the information today.
