63.2 Facilities & Programs.
An unannounced monitoring inspection was conducted from 3:00pm to 4:10pm. There were 53 children present with 5 staff members. A total of 5 children's records and 3 staff records were reviewed. The children were observed arriving to the program, eating afternoon snack. going outside, playing in centers with legos and creating perler bead crafts, and reading books. 4 children's medications were reviewed. Standard #: 22VAC40-185-130-A Description: Based on record review, the center did not obtain immunization records for a child before the first day of enrollment. Evidence: Child # 1's first day of enrollment was 1/31/19. Child # 1's record did not have copy of the immunization records on file. Plan of Correction: We have requested the immunization record. We will follow up.
Standard #: 22VAC40-185-140-A Description: Based on record review, the center did not obtain a physical exam for a child enrolled within 30 days of enrollment. Evidence: Child # 1's first day of enrollment was 1/31/19. The center did not obtain a copy of the physical exam for Child # 1 within 30 days of enrollment. Plan of Correction: We will put in a second request to obtain a copy the physical exam.
