Comments:
A Subsidy Health and Safety Inspection was conducted on April 9, 2019 during the aftercare portion of the program. The Licensing Inspector arrived at 3:05pm and departed at 4:50pm. Upon arrival to the facility there were no children present with two staff members. Throughout the course of the inspection there were three staff members and 17 children. The licensing inspector observed the children transitioning to the program, having snack, and playing at various tabletop stations to include but not limited to creative arts, puzzles and games, science and homework and talking freely amongst themselves. Five children's records were reviewed. The licensing inspector observed the physical plant, emergency and non-emergency supplies and documentation, nutrition, required postings, supervision, and health and safety. The exit interview was conducted with the Area Manager. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-500-F
Description: Based on review, the vendor did not ensure that there was a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present. Evidence: The record for Child #2 was reviewed and it was indicated that the child had food allergies that the staff had not documented on the center's allergy list.

Plan of Correction: The staff will update the list everytime we have a new student.

Standard #: 22VAC40-665-500-E
Description: Based on observation and inspection of the facility, the vendor did not inform all staff who work with children of children's allergies, sensitivities, and dietary restrictions. Evidence: The record for Child #2 was reviewed and it was indicated that the child had food allergies that the staff were not aware of.

Plan of Correction: All staff will check enrollment forms and company allergy lists.

Standard #: 22VAC40-665-520-B-3
Description: Based on record review, in five of five children's records reviewed, the vendor did not obtain all required parent contact information. Evidence: There was no documentation of one or both parent's work addresses in all five children's records reviewed.

Plan of Correction: We will talk to each parent to make sure that we get work addresses.

Standard #: 22VAC40-665-650-E
Description: Based on observation and inspection of the facility, the vendor did not develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: The center had not developed and implemented a consistent care policy.

Plan of Correction: The policy is in a pending status and currently being developed.

Standard #: 22VAC40-665-780-A-3
Description: Based on review, the vendor did not ensure that lockdown procedures shall be practiced at least annually. Evidence: There was no documentation of a lockdown drill practiced in 2018.

Plan of Correction: A lockdown drill will be completed tomorrow.
