Comments:
An unannounced monitoring inspection was conducted today. There were 58 children in care, with a total of 13 staff members assigned to their direct supervision. Children's records, staff records, background checks, staff training documentation, emergency preparedness items, classroom programming, children's medications, and the indoor/outdoor physical plant were inspected today. The children were observed playing on the playground, enjoying an Easter egg hunt, attending library sessions, painting handprint bunnies, enjoying various open center activities with a large variety of age-appropriate and theme-based activities, and practicing songs for their upcoming Mother's Day tea performance. This inspection was completed between approximately 10:10am and 11:50am. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, it was determined that one record was not complete. Evidence: The file for substitute staff person A does not include a second reference check.

Plan of Correction: I will do the reference check this week.

Standard #: 22VAC40-185-240-A
Description: Based on a review of staff training documentation, it was determined that one staff person has not completed all required orientation training. Evidence: Substitute staff person A has not completed First Day Orientation training.

Plan of Correction: She has only worked here 2 days this school year because she is a substitute. I will do the training with her today before she leaves.

Standard #: 22VAC40-185-330-B
Description: Based on observations, it was determined that various fall zone areas of the playground do not have an adequate level of resilient surfacing. Evidence: There is not enough mulch in the fall zone surrounding the caterpillar climber and both of the rocking riders.

Plan of Correction: I have already contracted for the mulch to be delivered and installed while we're closed for spring break.
