Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on April 4, 2019 from 3:00 p.m. through 5:15 p.m. There were thirty-four children in the care of four staff. Five children's records and five staff records were reviewed. The medication and medication paperwork was reviewed for one child. Areas of non-compliance may be found on the violation notice. Areas observed were: outside play, hand washing/bathroom procedures, snack, homework and inside play. Interactions between the children and staff were observed as positive and the children seem comfortable with the daily routine and schedule. Description: Based on a review of the background checks for persons designated as applicants/agents representing the business entity, and interviews with administrative staff, there was one representative with missing background check information and who did not submit to fingerprint requirements that were implemented on January 22, 2018 for the results of a criminal history record check that was due by September 30, 2018. Evidence: 1. Board Officer #1 became a business entity representative January 1, 2018 and the criminal history record check was not completed until February 27, 2019. 2. Board Officer #1 did not have documentation of a signed sworn statement. 3. Board Officer #1 did not have documentation of obtaining a central registry search within 30 days of becoming a business entity representative on January 1, 2018

Plan of Correction: Documentation of all background checks have been completed and viewed by the licensing inspector.
