An unannounced mandated Monitoring inspection was conducted today from 4:30 to 6:30pm. There were 47 children present with 8 staff. Children were observed playing uno, playing wii, creating beaded art projects, playing in the sensory room, and reading a book. Reviewed 5 children's files, 4 staff files, emergency evacuation log, injury log, attendance, medications and authorizations, and physical plant. Standard #: 22VAC40-185-140-A Description: Based on record review, children did not have a physical examination within the first 30 days of care. Evidence: Child#3 and Child#4 did not have documentation of a physical. Plan of Correction: I will ask the parents for a copy.
Standard #: 22VAC40-185-60-A Description: Based on record review, children's records were incomplete. Evidence: Child#1 and Child#3 did not have documentation of annual child updates and confirmation of up-to-date information in the child's record. Last update was at enrollment- 8/14/17, 9/8/15 Plan of Correction: I will have them update the information
