Comments:
An unannounced monitoring inspection was initiated today. The physical plant, 5 staff files, 5 children's files, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 11:50-1:20pm. There were 38 children under the supervision of 5 staff. During the inspection, children were observed reading books, eating lunch, involved in group activities, enjoying free play and preparing to rest during naptime. For lunch, the children were served spaghetti with bolognese sauce (with chicken), corn, oranges and milk. The interactions between the children and staff were seen to be positive and constructive in nature. Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-240-C
Description: Based on file review and interview, 5 out of 5 staff have not completed a minimum of 16 hours of annual training. Evidence 1. All 5 staff records do not have documentation of having received 16 hours of training for 2018.

Plan of Correction: We are working to meet this requirement for 2019.

Standard #: 22VAC40-185-270-A
Description: Based on observation, there was an area of the center which was not maintained in a safe condition. Evidence 1. The fence that surrounds the playground has a wooden slat which has broken off. This created a sharp edge which could entangle clothing or skin or be an entrapment hazard. There is also a section of the fence (near the building) that has become detached, which exposes protruding nails/screws.

Plan of Correction: A work order will be placed for this to be fixed.
