Technical Assistance:
Discused daily schedules, allergy lists, child abuse and neglect trainings, and injury report forms.

Comments:
An unannounced renewal inspection was conducted today from 10:40am-1:45pm. There were 46 children (2-5yrs.) supervised by 6 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed singing songs, painting, journaling, making a list of words that begin with the letter "Z", and doing letter "Z" tracings. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, the center did not ensure that injury prevention procedures were updated annually based on documentation of injuries and a review of the activities and services. Evidence: The center did not have documentation of annual injury prevention procedures.

Plan of Correction: We will type up an annual injury prevention plan.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, 4 of 5 children's records did not contain documentation of all of the required information. Evidence: 1. Child #1 (start date: 9/1/2017) and Child #4's (start date: 1/8/2019) records did not contain documentation of either parent's work telephone number. 2. Child #2's (start date: 9/11/2017) record did not contain documentation of mom's work telephone number, and documentation of child updates and confirmation of up-to-date information in the child's record as required by 22 VAC 40-185-420 E 3. The most recent documented parental acknowledgement of review of emergency contact information was dated 9/11/2017. 3. Child #5's (start date: 9/4/2018) record did not contain documentation of dad's work telephone number.

Plan of Correction: I will contact parents today to get the missing information.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, 1 of 3 staff records did not contain documentation of all of the required information. Evidence: Staff B's (date of hire: 9/1/2017) record did not contain documentation of the employee's highest level of education.

Plan of Correction: I will request it today.

Standard #: 22VAC40-185-270-A
Description: Based on observation, 2 of 2 classrooms did not maintain areas and equipment of the center in a clean, safe, and operable condition. Evidence: 1. In the Daycare (children ages 2-3 years old) classroom, areas of chipped paint and cracking wood were on the window sill in the children's carpentry area. 2. In the PreKindergarten (children ages 4-5 years old) classroom, an area of chipped and peeling paid was on the window sill by the back door.

Plan of Correction: We will sand and paint the affected areas.

Standard #: 22VAC40-185-280-B
Description: Based on observation, 2 of 2 classrooms contained hazardous substances that were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the Daycare (children ages 2-3 years old) classroom, a can of lysol spray was in an unlocked cabinet in the children's bathroom. A spray bottle of bleach and water solution was sitting on top of the unlocked cabinet in the children's bathroom. 2. In the PreKindergarten (children ages 4-5 years old) classroom, a container of Comet cleaning chemicals was inside of a cabinet in the girl's bathroom. The safety lock on the cabinet containing the Comet cleaning container was broken.

Plan of Correction: We will maintain all chemicals in locked cabinets.

Standard #: 22VAC40-185-510-A
Description: Based on a review of medications, 1 of 2 prescription medications did not contain all of the required documentation. Evidence: Child #2's (start date: 9/11/2017) Epinephrine had a corresponding medication authorization form that was not signed by a parent or guardian.

Plan of Correction: We will ask parents to sign the authorization form.
