Evidence: The wallpaper located in the beginners classroom to the left of the perimeter door is peeling in two areas. The bolts on the boundary bumper on the Pre-K playground are protruding approximately 2 1/2 inches from the bumpers presenting a tripping hazard. A screw located in the molding of the Beginners left side of the wall is protruding from the molding presenting a hazard. Repeated from the November inspection: The bottom right corner of the perimeter door off the Toddler classroom is rusted and has peeling paint. This remains uncorrected from the last inspection.

Plan of Correction: We will repair all of these areas.

Evidence: Licensing Inspector observed Staff #1 changing a soiled crib sheet after an infant vomited. Staff #1 failed to clean the mattress with soap and water. Staff #1 used a cloth with bleach solvent to wipe the mattress.

Plan of Correction: We will retrain staff of this process of first cleaning and then sanitzing these surfaces.
