An unannounced monitoring inspection was conducted today April 3, 2019 to determine the center's compliance with licensing standards. The inspector initiated the inspection at 9:30 am and concluded the inspection at 12:15 pm. The assistant director and regional director participated with the inspection. Upon the inspectors arrival the toddlers and two year old classroom were combined. Children were singing songs and reviewing the months of the year. Children also worked on identifying numbers that were posted on paper flowers by counting the pedals. Preschool children were observed playing in centers (puzzles, blocks, doll house and chain link patterns). Later children had circle time reviewing the weather, singing songs and listening to a story. Today's activity consisted of cutting out ants and gluing them onto the letter "A". The infant classroom was observed during lunch. The school age children were in attendance during the inspection as it was spring break for the public schools. Children have been participating in a "minute to win it" challenge all week. Today's challenge was to move M&M's from one plate to another using a straw. Staff interacted in a positive manner and participated in activities with children. Today's lunch consisted of chicken nuggets, carrots, oranges and milk. All areas of the facility were inspected including the playground. The facility has age and stage appropriate materials and supplies. The first aid kit and non-medical emergency supplies were inspected and found complete. All required information was posted to include the license, last inspection notice, emergency numbers, menu, evacuation maps/procedures and daily schedules. The inspector interacted with staff and children in care. Six staff records and four children's records were reviewed. Two additional staff records were reviewed as a follow up. Currently medication is not being administered. Staff have current CPR, First aid and daily health observation training. The most recent evacuation drill was conducted on 03/20/2019. The two required shelter-in-place drill for 2018 were completed on 03/12/2018 and 09/07/2018. Two shelter-in-place drills will need to be completed by the end of the year. Today's Ratios Observed: Cat 2: (Toddler and 2 years) 9 children with 2 staff. Butterflies: (3-5 years) 13 children with 2 staff. Cuddle Bugs: (Infants) 7 children with 2 staff. School age children: 6 children with 1 staff. Corrections were reviewed for violations cited during the previous inspection. There were no repeat violations cited during today's inspection. Standard #: 22VAC40-185-140-A Description: Based on review of children's records, the facility did not obtain documentation of a physical for each child enrolled before the child's attendance or within one month after attendance. Evidence: 1. The record of child #1, enrolled 10/03/2017 did not have documentation of a physical exceeding 30 days from the first day of attendance. 2. The record of child #4, enrolled 03/12/2018 did not have documentation of a physical exceeding 30 days from the first day of attendance. Plan of Correction: Per the assistant director: I will contact parents and ask them to provide a copy of their physicals.
Standard #: 22VAC40-185-260-A Description: Based on review of documentation, the facility did not obtain an annual fire inspection report. Evidence: Documentation indicated the most recent fire inspection was conducted on 02/20/2018, exceeding the annual requirement. Plan of Correction: Per the assistant director: We will call the Fire Marshall again as a follow-up to schedule a day for an inspection.
Standard #: 22VAC40-185-440-H Description: Based on observation, the facility did not ensure cribs meet all requirements. Evidence: In the Infant classroom three of the four cribs had more than one inch of space between the mattress and the crib. Plan of Correction: Per the assistant director: We will order new mattresses for the three cribs that fit snuggly in the crib.
Standard #: 22VAC40-191-60-C-2 Description: Based on interview, the facility did not obtain a central registry finding within 30 days of employment. Evidence: 1. Staff #6, hired 02/14/2019, did not have a central registry finding on file. 2. Staff #6 reported a central registry had not been completed under this business entity. Plan of Correction: Per the regional director: A central registry will be requested and obtained immediately.
