Technical Assistance:
Technical assistance was provided regarding crib requirements for infants and it was discussed that hanging toys cannot be in cribs with infants older than 5 months or if they can crawl on their hands and knees. Requirements for proof of identification were also reviewed.

Comments:
An unannounced monitoring inspection was conducted today from 9:35am-2:00pm. There were 78 children directly supervised by 10 staff and all classrooms were in ratio. The physical plant, 7 staff records, 7 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed reading books, playing outside, singing, celebrating a birthday, and eating lunch which consisted of beef meatballs, egg noodles, mixed vegetables, apples, and milk. Positive interactions between children and staff were observed during this inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-130-A
Description: Based on record review, the center has not obtained documentation that each child has been adequately immunized before attending the center. Evidence: 1. Child #6 (Start date: 3/18/19) was present at the center today and does not have immunizations on file.

Plan of Correction: I will request the immunizations from the parent today.

Standard #: 22VAC40-185-160-C
Description: Based on record review, 1 of 7 staff records reviewed did not contain documentation that a Tuberculosis screening or test has been completed every two years from the date of the first initial screening or testing. Evidence: 1. Staff #5's most recent Tuberculosis test on file was completed on 1/20/2015.

Plan of Correction: This staff has had a TB test in the past 2 years and will go to the Health Department to get a new copy.

Standard #: 22VAC40-185-60-A
Description: Based on record review, 5 of 7 children's records reviewed are not complete. Evidence: 1. Child #1 and Child #2's records are missing the parental agreement which includes the authorization for emergency medical care, agreement for parent to pick up their ill child, and the agreement for the parent to notify the center if the child or family has a communicable disease. 2. Child #4's record is missing the father's work phone number. 3. Child #6's record is missing the father's place of employment and work phone number. Child #6's record is also missing documentation that proof of identification was verified within 7 days of the child's start date. Child #6's start date was 3/18/19. 4. Child #7's record is missing the mother and father's places of employment and work phone numbers.

Plan of Correction: I will obtain this information from the children's parents.

Standard #: 22VAC40-185-240-D-5
Description: Based on record review and staff interview, there is not at least one staff member who has obtained within the last three years instruction in performing the daily health observation of children. Evidence: 1. The director stated no one has completed daily health observation training since March 2016.

Plan of Correction: We will get everyone trained in daily health observation.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas inside the center were not maintained in a clean and safe condition. Evidence: 1. In the bathroom next to the 3's Classroom, chipping paint was observed above the cove base at the bottom of the wall. Chipped paint and drywall material that has flaked off the wall was laying on the floor below.

Plan of Correction: I will call the rental office and they will come and fix it.

Standard #: 22VAC40-185-500-B
Description: Based on observation, a foot operated disposal system was not in place for the disposal of soiled diapers. Evidence: 1. In the "2's and 3's" Classroom, the foot operated trash can was broken preventing the opening of the trash can lid using the foot pedal.

Plan of Correction: I will buy a new foot operated trash can today.

Standard #: 22VAC40-185-520-C
Description: Based on observation, a diaper cream was not labeled with the child's name. Evidence: 1. A diaper cream in the Infant room was not labeled with the child's name.

Plan of Correction: The diaper cream has been labeled. I will remind everyone that diaper creams are always labeled.

Standard #: 22VAC40-185-560-F
Description: Based on observation and staff interview, the center has not posted or made available to parents the menu for the current week. Evidence: 1. A current posted menu was not found in the center. 2. The director stated that she has not made a menu for this week yet.

Plan of Correction: I will make the menu today and post it.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation of the physical plant, the findings of the most recent inspection of the facility were not posted. Evidence: 1. The most recent Violation Notice from November 2018 was not maintained at the center.

Plan of Correction: Today's Violation Notice will be posted.
