A Subsidy Health and Safety inspection was conducted on (insert date) to review supplemental health and safety requirements. A total of 78 children were present with 10 caregivers. The children were engaged in the following: reading books, playing outside, celebrating a birthday, and eating lunch. Programming, menus, emergency procedures, and 7 children?s records and 7 staff records were reviewed. The director advised no medication was on site. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: 22VAC40-665-580-B Description: Based on record review, 3 of 7 staff records reviewed did not contain documentation that the Virginia Preservice Training for Child Care Staff was completed within 90 days of employment. Evidence: 1. Staff #2 (Date of Hire: 3/16/09), Staff #3 (Date of Hire: 7/1/96), and Staff #4 (Date of Hire: 10/8/14) did not have copies of their Virginia Preservice Training for Child Care Staff certificate on file. Plan of Correction: We will obtain copies of the training certificates.
Standard #: 22VAC40-665-780-A-3 Description: Based on staff interview, the center has not practiced lockdown procedures at least annually. Evidence: 1. The director stated she was unaware of this requirement and has not completed a lockdown drill yet. Plan of Correction: We will do a lockdown drill this afternoon.
