The licensing inspector conducted an unannounced renewal inspection on Tuesday, April 16, 2019 from approximately 12:10 PM until 4:00 PM. The following ratios were observed with a total of 71children present during the inspection: Infants - 7 children with 2 staff Toddlers - 9 children with 2 staff Twos/Threes? 15 children with 2 staff Threes/Fours - 15 children with 2 staff School Age - 25 children with 2 staff During the inspection, the children were observed resting at nap time, listening to music, having books read to them, washing their hands and taking bathroom breaks, eating snack and playing with toys. Areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The center was equipped with toys and supplies, and items were available to the children. The provider stated transportation is not provided at this time. Medication is administered and medication, authorization forms and allergy plans were reviewed. During the inspection, six children's and six staff records were reviewed. The following information was reviewed: emergency supplies, injury reports, injury prevention plan, fire, lock down and shelter-in-place drills, daily attendance, infant daily sheets, parent agreements, liability insurance policy, required postings and staff training information. Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not ensure all areas and equipment were maintained in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed two playground timbers on the Toddler playground that were deteriorating which created splintered edges. Plan of Correction: I will have the timbers replaced with new ones.
Standard #: 22VAC40-185-330-B Description: Based on observations and measurements, the center did not ensure the required resilient surfacing was located in the fall zones areas. Evidence: During the inspection of the Toddler playground, one plastic slide that was approximately two and a half feet tall had approximately one inch of pea gravel at the end of the chute. Six inches of pea gravel would be required in all fall zone areas around the slide. Plan of Correction: I will remove that piece of equipment from the playground.
