Comments:
An unannounced monitoring inspection was conducted on 4/16/19 from 10:00am -12:00pm. When the Licensing Inspector arrived there were 48 children ages six months old to ten years old in care with 13 staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch and during rest period. Records were reviewed for five children and five staff. There was no medication at the center. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that the facility did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1. There was a section of the exterior fence on the playground that has damage from where a car backed into it. A couple of the metal bars are bent and one section has a crack in the metal. 2. Staff #6 (Program Director) confirmed that the fence was not currently in a safe and operable condition.

Plan of Correction: The facility responded: We were aware of the damage and the fence will be repaired.
