We discussed the requirements for resilient surfacing and staff records. The risk assessments for violations were provided. As a reminder, the criminal history record finding completed with the Office of Background Investigations includes the VA State Police background check and the former method of obtaining that background check is not required.
Thank you for your assistance during today's unannounced monitoring inspection conducted from 10:28 AM to 2:50 PM. Today, there were fifteen children, ages three through five years, with two teachers. Ten children remained for the afternoon with two staff. I viewed program activities, equipment, daily routines, posted information, asbestos surveillances, fire inspection report, emergency supplies and drill records, injury records, lunch, five records for children, six staff records, staff qualifications, staff training, indoor and outdoor areas. The children enjoyed an egg hunt followed by a discussion of the animal pictures from the eggs and egg hatching. The children enjoyed access to a variety of learning materials and interest areas for play. Stories were read to the children, including a time with a "mystery reader". The teachers have provided regular communication with families.
Standard #: 22VAC40-185-70-A Description: Based on a review of six records for staff, and interviews with administrative staff, there was information missing from two staff records. Evidence: There were not any copies of college transcripts or applicable degrees to demonstrate that staff member 5 possesses the education required for the job position held. Plan of Correction: An administrative staff person will obtain a copy of the college transcripts or diploma for the record of staff member 5.
Standard #: 22VAC40-185-330-B Description: Based on the measurement of the depth of resilient surfacing at the fall zones of playground equipment, and an interview with staff, the surfacing did not comply with minimum safety standards. Evidence: The depth of the pea gravel at the base and side fall zones of the Little Tikes red slide (beige composite structure with a height of thirty inches) was two to three inches and not a minimum of six inches. Plan of Correction: The teacher will make sure the depth of the pea gravel is replenished by raking material into the fall zones. The teachers or school maintenance staff will regularly check on the need to rake the gravel and replace pea gravel in fall zones.
